HemophiliaBumps and scrapes are a part of every child's life. For most children, a tumble off a bike or a stray kick in a soccer game means a temporary bruise or a healing scab. But for children with hemophilia, these normal traumas of childhood are reason for extra concern.
Hemophilia is a rare bleeding disorder that prevents the blood from clotting properly. Currently, about 17,000 people in the United States have hemophilia. About one in every 8,000 boys is born with hemophilia; girls are more rarely affected by this genetic condition linked to gender. A male can't pass the gene for hemophilia to his sons, though all his daughters will be carriers of the disease gene. Each male child of a female carrier has a 50% chance of having hemophilia.
What Is Hemophilia?
Human blood contains special proteins, known as clotting factors. Identified by Roman numerals, clotting factors help stop bleeding and allow a blood vessel to heal after an injury. The last step in the clotting process (also called coagulation) is the creation of a ';net'; that closes the torn blood vessel and stops the bleeding. This part of the process involves clotting factors VIII and IX. People with hemophilia are deficient in one of those factors due to their disease genes, and as a result, their blood can't clot properly.
Hemophilia A, also known as factor VIII deficiency, is the cause of about 80% of cases. Hemophilia B, which makes up the majority of the remaining 20% of cases, is a deficiency of factor IX. Patients are classified as mild, moderate, or severe, based on the amount of factor present in the blood.
Each case of hemophilia is unique. A patient whose blood tests suggest severe hemophilia may only bleed occasionally, whereas another patient with a milder form may bleed more often. The reasons for this variability may relate to other clotting factors or to differences in behaviors that present different risks of injury.
Signs and Symptoms
Signs and symptoms of hemophilia vary, depending on severity of the factor deficiency and the location of the bleeding. Few babies are diagnosed with hemophilia within the first 6 months of life because they're unlikely to sustain an injury that would lead to bleeding. For example, only about 30% of males with hemophilia bleed excessively when circumcised and only 1% to 2% of newborns with hemophilia have bleeding within the skull (called an intracranial hemorrhage).
Once babies with hemophilia begin crawling and cruising, parents may notice raised bruises on the stomach, chest, buttocks, and back. Sometimes, because bruises appear in unlikely places, parents may be suspected of child abuse before their child is diagnosed with hemophilia.
The baby may also be fussy and may not want to reach for a cup, walk, or crawl. Other symptoms include:
prolonged nosebleeds
excessive bleeding from biting down on the lips or tongue
excessive bleeding following a tooth extraction
excessive bleeding following surgery
blood in the urine (called hematuria)
The most common type of bleeding in hemophilia involves muscles and joints. A child with hemophilia will usually refuse to move the affected joint or muscle because of pain and swelling. Recurrent joint bleeding can also lead to chronic damage.
Diagnosis
Your child's doctor may suspect your child has hemophilia if there's a pattern of bruising and bleeding, particularly if this includes bleeding into the joint. Diagnosing the condition requires a set of blood tests, including a complete blood count (CBC), prothrombin time (PT), activated partial thromboplastin time (PTT), factor VIII level, and factor IX level.
Treatment
Although hemophilia is a lifelong condition with no cure, it can be successfully managed with clotting factor replacement therapy - periodic infusions of the deficient clotting factor into the child's bloodstream. Factor replacement may be given through an intravenous (IV) line either at the hematology clinic or at home by a visiting nurse or by parents (and even older patients) who have undergone special training. Your child's hemophilia team (doctors called hematologists who specialize in treating blood disorders, nurse practitioners, nurses, and social workers) will teach you how to prepare the concentrated clotting factor and when and how to inject it into your child's vein. Once the clotting factor is ';infused,'; it begins to work quickly and helps prevent joint damage.
Although these treatments are effective, they are also expensive. According to the National Heart, Lung, and Blood Institute, most children in the United States who begin receiving regular infusions early in life will exhaust the average lifetime insurance limit of $1,000,000 by their second decade.
Between 14% and 25% of children with severe hemophilia A develop inhibitors (antibodies to the clotting factor). Their bodies view the clotting factor as a foreign substance and develop inhibitors that block its clotting action. This can make the hemophilia difficult to treat. One method for overcoming the inhibitors is to increase the body's tolerance to the clotting factor by carefully infusing increasing amounts of the clotting factor over time. Inhibitors to factor IX (hemophilia B) are less common.
Also, a new medication called recombinant factor VII has helped many patients with inhibitors. It activates another part of the coagulation process directly and bypasses the deficiencies.
Preventing Problems
You can help your child with hemophilia prevent problems by encouraging healthy behaviors now. For example, exercise can strengthen your child's muscles and help decrease bleeding from injuries. Swimming is strongly encouraged because it exercises all the muscle groups without putting stress on the joints.
Your child's weight should also be managed properly, because excess weight can cause strain in regions of the body. If your child is overweight, speak to your child's doctor for advice on weight management.
Medications can also help prevent problems in children within hemophilia. Many patients prevent ';bleeds'; by infusing clotting factors on a regular basis (usually two or three times per week). Some young children have a surgical procedure to implant a central venous catheter (a hollow, soft tube) into a vein. The catheter can be used to give concentrates of clotting factors.
Your Child's Needs
Although each stage of development comes with its own set of issues, experts say the toddler and teenage years can be the most challenging for a child with hemophilia. Both phases naturally involve a child's quest for independence. For example, a toddler may not tell his or her parents about an injury that resulted from doing something that wasn't allowed (i.e., riding a bike without a helmet, jumping on the furniture, running in the house, etc.). Most children, though, will discover that seeking prompt treatment is better than waiting until pain and swelling become unbearable.
Ask your family members, caregivers, and your child's teachers if they would like to learn more about hemophilia by meeting with your child's doctor or other members of the hemophilia team
You should also encourage your child to still participate in activities, which may mean that he or she has to take on a different role at times. For example, if hemophilia prevents your child from participating in a contact sport, he or she can still be a part of the team as the scorekeeper or assistant manager.
Another excellent way to help children with hemophilia cope is to send them to an appropriate summer camp. Several camps provide children who have bleeding disorders with an opportunity to meet other kids with the same condition, as well as to work toward being able to give themselves clotting factor replacement therapy, which can give them a sense of control over hemophilia. Ask your child's doctor for information about finding a camp near you.
Caring for Your Child With Hemophilia
When your child is an infant, you should put bumper pads in the crib, cushion furniture with sharp edges, and put gates across stairs to prevent falls. As your baby begins to crawl and walk, special knee and elbow pads can offer protection against joint bleeds. Some parents sew a pocket in the seat of their child's pants and pad it with a piece of diaper. If your house has ceramic tile or hardwood floors, consider installing carpet or buying rugs to soften the floor surface.
Depending on how rambunctious and adventurous your toddler is, you might want to have him or her wear a helmet to protect against head injuries.
Dental care is just as important for a child with hemophilia, but routine cleanings can sometimes cause bleeding. For this reason, it's important that your child's dentist has experience with hemophilia. This will make it easier for the dentist to respond to any bleeding.
Children with hemophilia can generally sense when a bleed has occurred. They often describe a tingly or bubbly sensation in a joint. It may also feel warm to the touch. Encourage your child to tell you when he or she senses a bleed - a quick infusion is the key to preventing long-term damage.
Doctors also recommend splinting an affected joint for a short period of time and then applying ice to decrease inflammation, promote clotting, and relieve pain. Acetaminophen (such as Tylenol) is the preferred pain reliever because many other over-the-counter pain medications contain aspirin or NSAIDs (non-steroidal anti-inflammatory drugs such as ibuprofen or naproxen sodium), which can affect blood platelets and lead to increased bleeding.
Bleeds must be treated promptly because prolonged bleeding can cause joint disorders. The accumulation of blood in the joint spaces can erode the smooth surfaces that allow limbs to bend easily. As the surfaces roughen, inflammation and the number of bleeds can increase. This cycle can lead to chronic joint damage that may require surgery to remove the damaged joint tissue (called a synovectomy).
When to Call Your Child's Doctor
Certain bleeds require medical attention, including those injuries affecting:
the central nervous system - any suspected trauma to the head, neck, or back
the face, including the eyes and ears
the throat or another portion of the airway
the gastrointestinal tract (which might produce signs such as bright red or black blood in your child's stool)
the kidneys and urinary tract (if you find blood in your child's urine, this may require treatment and bed rest)
the iliopsoas muscle in the trunk (which might produce signs that mimic a hip or abdominal bleed, including lower abdominal/groin or upper thigh pain, an inability to raise the leg on the affected side, and a feeling of relief when contracting or flexing that side of the body)
the genital area
the hips or shoulders (these can be complicated bleeds because they involve the rotator joints)
large muscle compartments, such as the thighs
If the bleed requires going to the emergency room, make sure your child is treated at a hospital that has experience treating hemophilia. Any injury affecting the brain or any part of the central nervous system or a vital organ should be treated as an emergency and you should get medical assistance immediately.
Looking to the Future
Tremendous advances have been made in the treatment of hemophilia, and most patients can now lead full, healthy lives with careful management of their condition.
The development of clotting factors made in the laboratory has virtually eliminated the danger of transfusion-related infection with HIV or hepatitis viruses from clotting factor replacement therapy. And regular home-based infusions have helped reduce chronic joint problems.
In the future, people with hemophilia may have access to continuous infusion of clotting factors under the skin or in pill form. Some doctors are also encouraged by research involving gene therapy.
Thanks to advances like these, children with hemophilia can now participate in a wide range of sports and have the freedom to lead more active lives.

Some advise for connection speed on xbox live?
ok, so, i have comcast with high speed internet, port forwarding and a wired connection. every time i log on to call of duty 4 it says that i have 2 or 3 bars. this is enough to make me lag so bad that i cant get kills. by the time i see them they're already shooting me. i have tried all obvious solutions including NAT . Pretty much no matter what i do it still lags.Some advise for connection speed on xbox live?

No, if you give it orally she will get diarrhoea. If you use it on her nipples she will lick it off and could get diarrhoea.

However in some cases where the ***** appears physically ill or the behavioural changes are of such magnitude as to cause concern, treatment has to be instituted. Not a good idea. But you could help her mammary glands to produce less milk by reducing stroking, wiping and not allowing her to lick as all these things encourage the glands to produce milk.

African grey bathing advise required please?
My boyfriend has had an african grey for about a year and a half and he is about 2 1/2 years old now. We have noticed that whenever he gets a bath (is he bathed in the bathtub) once he is back in his cage he always wants to put his feet in his water and seems slightly agitated. Is this something we should be worried about? I have tried searching through websites but can't seem to find anything about another african grey doing this.African grey bathing advise required please?

Once they are wet, they want to keep playing, so as soon as they get back to the cage they finish bathing in their drinking water. Mine dips its tail feathers, head and feet in and makes a general mess of the entire room. I've tried to give her a bathing bowl on the cage bottom but it's not the same, she wants to bathe in her water bowl.

How do i advise a friend?
she had a boyfriend who insisted of having sex without using a condom ,so to avoid getting pregnant she started using the pills .When she stoppped using the pills she has been having heavy virginal excreations and they were diffrent in colour .She recently went to hospital and was told to see a gynacologist something she really is opposed to .She claims it is just a side effect from using the pills but im fearing she could be infected any gynacologist reading this please tell me could this be a serois case which my dear buddy is just joking around with .what would you reccomend me to do coz i've tried to convinse her go to see a gynacologist but she just snobbs me and thinks im trying to create a mountain from an anthill!How do i advise a friend?

I'm sorry I can't help more but she really does need an exam. Maybe you can convince her to see your GYN or you could go with her for support. Maybe she is just afraid of a vaginal exam because she's never had one before? Just reassure her (without sounding like you're attacking or insisting) and offer to go to the doctors with her.
If your friend doesn't have insurance, there are places such as the Health Department or Planned Parenthood. There is always a way to get a free GYNO exam.

In fact, many people look for saudi transfers at the company they work for (mostly energy, aviation, engineering, and financial ) and either have the choice of living outside the compound (which I dont reccomend if you dont know arabic, although almsot everyone now speaks satisfactory english, and every road sign is in both languages, its just easier for foreigners to stay in an all-in-one compound) or in a compound (Saudi Aramco has one of the largest compounds in saudi arabia. Free Villa housing, paid utilities, and security is provided by the National Guard{its a part-government asset].
So it is very safe, and for your partner, there are countless foreign families in the compound and outside. Shuttle Buses are available in all compounds to take you to one of Jeddah's or Riyadh's (if you are there) mammoth size, double jumbo malls. American malls are probably 1/100th the size and quality. Btw and FYI, there are on average 3 to 4 multi-billion dollar shopping malls opening every year or so.

Aramco also has a private beach that's only for employees. It's very clean and you can swim there. Aramco really does a good job to make sure their employees are comfortable and very well taken care of. It's very safe inside the compound....you see kids out playing alone, riding their bikes in the streets, etc. you see people out walking all the time. It's sorta like a dream community...the kind of place you feel safe to let your kids out alone....
I lived with my husband and sons in Yanbu, Saudi Arabia fo r5 years. Had a wonderful experience. That said, you absolutely cannot go into it expecting it to be like home. It is not. Women cannot drive, go into barbershops, be in a car or house with a man not her husband, father or male relation.
We did not live on a compound, and i refused to be holed up in my house. I taught in the International school, met women and families from all over the world and even ate dinner in the homes of Saudis.
But I did not spend my whole time ';Saudi bashing'; and complaining.
We were there to make money and we did. We did our best to be respectful and we never had any problems.

idk if i can help or not i play the guitar and i had alot of trouble learning notes and multiple things so what i do to help my self learn is id figure out what i had most trouble with witch in your case is lower notes and id make up my own practice for you the lower f# add a few others and just play them over and over and over don't worry about how you sound that doesn't matter if you keep at it sooner or later it will sound better and you'll get better you find out what your doing wrong and in the process you get better at it each time with ease im not 100% sure this will help but this is what i did on the guitar/drums but the guitar and sax are alot different i hope i helped in some way ^^
lower your jaw, try to blow more warm (slow) air. think 'aooowww' a low deep drone. relax those lips!

However, since it is only an occasional problem, we just use cleaning supplies and an odor neutralizer to combat the smell. I have heard stories from a lady who has been working there since the 70's and they had a patient who constantly did this. They ended up redoing the whole room after he was moved to a psychiatric ward because the smell was so bad.
I know I didn't help you, but maybe you should talk to your local hospital about the problem and they may have some suggestions.

With any form of art you have to go to your studio and create, destroy and create again. It's always about trying things out, using different primers, using a different mixing agent, and different surface.
Sometimes an unprimed canvas can give a really nice quality to the work.
Gesso is a white liquid that you should paint onto your painting surface before you paint, it stops the paint from soaking into the medium and losing its colour. I suggest if you paint with oils to buy the good stuff %26amp; be heaps precise.

Try not to dwell so long on the vaccinations. I think I psyched myself out thinking about it. The way we were given our shots, we didn't see how they were doing it until we were right at the door.
Like everyone else said, start your exercising now. Try to get to the standard fitness level that you'll need to pass.
Try to get along with the people in your flight and stay the Hell away from the drama. (This is probably obvious)You do not want to have enemies. I've seen and heard of people being sabotaged by their fellow flight mates. It's a pathetic thing to do, but unfortunately some people your age just lack the civility to handle situations.
I went through BMT for a different branch, so I don't know when they actually issue you your uniform, but I would wear old shoes just in case they have you march somewhere. I wore some boots that aren't made for marching in(They had really deep grooves on the soles) and the heels were pretty much destroyed from doing facing movements on the road.

PT in the Navy isn't hard unless you don't know how to run or can't do situps or pushups. As for the shots - they are done in less than a minute depending on what you have to get. The only one that really sucks is the ';peanut butter'; shot. (It really does feel like they injected you with ';peanut butter';.) Don't psyche yourself out! Try to get a job on Div staff! (You don't have to stand watches then - so more sleep for you!) Being away from home does suck and after 8 weeks you feel like you've been in RTC forever. But don't cry it's not worth tears or the yelling at you might get from your female RDC who does not believe in crying! Here's helpful hints:
- Know your 10 general orders inside and out: up and down right and left - its much easier if you know them all before you get there it saves on pushups and eight counts.
-Don't get involved in the DRAMA!!! Okay I cannot emphasize this enough - stay away from that and don't even bother trying to get a boyfriend or whatever - it isn't worth it.
-Learn your rank structure before you go so you can save yourself a lot of pushups by calling a Senior Chief, Chief or a Chief, Petty Officer or god forbid an officer, chief.
-And the most important thing is this: You enlisted to do a job. Don't bother trying to get a date or some loving. You are there to do a job, just do your job. There will be plenty of time to live through the drama and all that jazz in the fleet. Just get out of boot camp.
PS - Don't go to sick call if you can help it. It'll set you back, and drink lots of milk - it's good to prevent shin splints and stress fractures. I'm talking about 3 glasses a day.
Just remember that your drill instructors are people too, don't take the things they say or do too seriously, and always remember that it is going to end soon enough, just keep your head down and your mouth shut, you'll do fine...
Follow instructions, don't stand out from the crowd, don't take anything the TIs say personally, you will probably get attitude from everyone. Don't take any of it personally. Be orderly with your stuff. Keep everything clean and neat.

I can't really say if its either or it just depends on the question. I mean some people are really looking for answers and I try to help them as much as I can.
On the other hand, there are stupid people that ask dumb questions and they're going to be answered accordingly.
Like this senario.
There is a 18 yr old asking how she can break up a family because she likes her male neighbor and wants him all to herself. She asks not to be judgmental and persist she's not a home wrecker because his kids are 19 and 20-something. In this situation I would be blunt and sarcastic maybe even intentionally rude.
But with a question like yours, where you need an honest answer I give you one and hope I helped you enough then go on my merry way.When you give out advise does it come more from you caring or you being judgmental?
Depending on the situation in which I am giving advice it's a little of both I guess. I don't mean to be judgmental but I guess in all honesty I sometimes am. Never in a negative way like some people. I just say what I believe and if the person still disagrees or decides to go in a different direction I give up. If one person doesn't go through what I have as a result of me helping them do it right, then going through what I have was worth it.
Most of the time, it comes from a desire to help, if I can... Occasionally, though, if I've just seen it asked too much, or if it's obviously troll-like, I can lose my patience, and be less-than-civil. :) (Plus, pregnancy hormones can get the better of me, at times.)
But, two things I don't like, are:
1.) When someone DEMANDS you answer them.
2.) When people only ask a question, cause they want to hear what they want to hear, and they, themselves, are not tolerant to their responder's opinions... And, then they give you a thumbs-down!
For me it always comes from me caring. I think answerers who come from a place of judgment and condemnation are very rude and have no reason to be hurtful or condescending to a stranger.
So even if some of my answers sound a bit gruff, they come from a place of pure love and caring and a desire to help. Always. :-)

She asks not to be judgmental and persist she's not a home wrecker because his kids are 19 and 20-something. In this situation I would be blunt and sarcastic maybe even intentionally rude.

Depending on the situation in which I am giving advice it's a little of both I guess. I don't mean to be judgmental but I guess in all honesty I sometimes am. Never in a negative way like some people. I just say what I believe and if the person still disagrees or decides to go in a different direction I give up. This way the premiums will be very low and at least you will have coverage in case something really big happens like cancer, surgery, etc.

go to sitges for the day, it is about a 30 minute drive and it is beautiful. It is a beach tow and the beaches are divided up for families, single/lesbian women, and singlegay men. Get the tourist bus. Their is a red and a green route you can hop on and off and switch routes. I can not remember if one day ticket does both but it is a good buy. I noticed that they Gaudi 's building and Picasso are already covered by others. there is something left though... FOOD... go to 4 GATS near Las Ramlas st. where Picasso used to hang out. they have a appetizer, the entry and the desert for a very reasonable price and a drink (wine or soft drinks). and the food tastes great .. you Will love it there... if you like shopping , there is a street in parallel with Las Ramblas , called something Angelese... it's on the right handof Las Ramblas.. full of great stores...don't forget to try Tapas....
they are the best in sea food. and also have CAVA with your feed.

Easiest thing to do is to do what I did. Get the Bus Turistic Open-Top bus from Placa de Catalunya at the top of Las Ramblas. It'll take you to every major tourist attraction in the city and really helps you to get your bearings. Tickets are valid for either 24 or 48 hours and its Hop-On Hop-Off as many times as you like.
La Sagrada Familia by Gaudi
There are many great place to visit in Barcelona, so as mentioned before the best thing is to get on the tourist bus (there are 2 routes, one north and the other south) you can buy tickets in the tourist office in Plaza de Catalunya or on the bus. There is a choice of 1 or 2 day tickets and to be honest I'd buy the 2 day ticket as there is so much to see. You can hop on and off all day and change buses. On top of the things to see would be the architecture which is very impressive, even if you're not cultural (Parc G眉ell, Liceu, Las Ramblas. La Pedrera and Sagragarda Familia amongst many!)
Then you should miss a trip to Camp Nou to the stadium and Montjuic is a great place to visit as well and the views are incredible! And if you've got time take a trip up to Tibidarbo which is incredible.
If you like shopping there is a large department store 'el corte ingles' in Plaza Catalunya. Also walk around the barrio gothico which has some great hidden shops, if you like designer clothes go to Paseo de Gracia. There are also lots of museums including the Picasso museum.
And don't forget to go to the beach! At night go to the Port Olympic which is full of bars and restaurants.

For sure the Barri Gotic around the cathedral, Parque Guell, La Sagrada Familia, Mont Juic. Best take a three day pass for the Turist Bus Lines (3 lines). Those busses take you to all points of interest. Don麓t miss the Fundaci贸 Joan Mir贸 with it麓s terrace cafe on the slopes of Mont Juic. Have a stoll en Las Ramblas.

If you got enough of the town, you can tak the train from Plaza Espnya to the coisters of Montserrat as a day trip or from Sants Station a trin to Tarragona with a lot of roman ruins.

Weel I went to the Parque G眉ell yesterday and think it's worth visiting. If you want to go to the Sagrada Familia, do it by night. In the Passeig de Gracia you'll find 2 houses from Gaudi : Casa Mila and Casa Batllo. I've been told that the casa Mila is great and much more cheaper (4 or 5 鈧?instead of 16鈧?.

Don't miss the Monastery at Montserratt, it's awesome. It's about 45k north of Barcelona inland and about another 40k north further on there is an 8th century castle which is now a Parador. It's well worth staying the night or just going up there to sit on their terrace and have a coffee. At least you will have visited a place with real history.
